We've rewritten our accessibility statement using the GOV.UK model, adapted for LocalGov Drupal sites. It's written in plain English and covers the third-party content (planning portals, payments) that councils usually forget.

Would anyone review it before we share it as a template?

Happy to review. Make sure the 'disproportionate burden' section has a review date, not just a reason.
